REPORT OF BIRTH / REGISTRATION OF THE BIRTH OF A FILIPINO CHILD BORN OVERSEAS
Four (4) original duly accomplished Report of Birth forms (available at the Consulate or online)
Birth Certificate of the Child duly authenticated by the Australian Department of Foreign Affairs and Trade (DFAT) and afterwards photocopied in quadruplicate. Philippine passport/s of Filipino parent/s
Registered Marriage Certificate or Report of Marriage of the parents (if married abroad)
A non-refundable fee to be paid either in cash, commercial bank cheque or postal money order payable to the “Philippine Consulate General”.
Notes:
1. Applicants must submit four (4) copies of all original documents.
2. Additional documents may be required by the Consulate, as appropriate.
3. If the parent has is a Filipino dual citizen, the parent must also submit his/her Certificate of Australian Citizenship and Filipino dual citizenship document.
REPORT OF MARRIAGE / REGISTRATION OF THE OVERSEAS MARRIAGE OF A FILIPINO CITIZEN
Requirements (bring originals for sighting):
Four (4) original duly accomplished Report of Marriage forms (available at the Consulate or online)

Filipino applicant's Philippine Birth Certificate
Spouse's passport or driver's license (if not a Filipino)
Four (4) passport-sized photographs each of husband and wife (separate photos) with plain white background, taken within the last 3 months. Photos should not show applicant in sleeveless or revealing attire, and eyeglasses should be removed.
If widowed, National Statistics Office (NSO)-authenticated Death Certificate of deceased spouse and the NSO-issued Marriage Certificate.
If previous marriage is annulled, the NSO-authenticated Marriage Contract, duly annotated.
If divorced, authenticated copy of divorce decree. It is important to note that for a divorce decree to be valid in the Philippines, the petitioner for divorce should be the non-Filipino spouse.
