TO ALL APPLICANTS: 1) If married abroad, Report of marriage is required even if applicant opts not to adopt her husband’s surname. 2) In all cases, the consular officer reserves the right to require submission of further documents or other requirements as may be deemed necessary. 3) Philippine Civil Registry documents (birth, marriage, death certificates) issued by the National Statistics Office (NSO) may be ordered online at www.ecensus.com.ph. 4) Applications to be lodged in person are accepted from 9:00 AM to 3:00 P.M., Monday to Friday except during Australian and Philippine holidays.
First Time Applicants / Renewals / Replacement / Amendments
FIRST TIME APPLICANTS
- Personal appearance of applicant if more than 8 years old. If less than 8 years old, personal appearance of either parent.
- Duly Accomplished Application Form.
- Report of Birth issued by the Philippine Embassy/ Consulate General where birth was reported.
- Three (3) identical copies of 4.5 cm x 3.5 cm high resolution photographs in photo paper (colored with plain white background) taken within the last 3 months. Photo should not show applicant in sleeveless and revealing attire; collared shirt is required. Eyeglasses should be removed. Male applicants should not be wearing earrings.
- Non-refundable fee of AUD 65.00 for 32 pages and AUD 78.00 for 64 pages in cash, commercial bank cheque or postal money order payable to the Philippine Consulate General in Sydney.
RENEWALS
Note: An application for renewal of Philippine passport may be filed in person, by mail or by a member of the immediate family of the passport applicant authorized in writing by the latter. If applying by mail or through a representative, a Justice of the Peace must certify that the signature, thumb marks and photos on the application form are those of the applicant.
- Duly accomplished application form.
- Latest passport and photocopy of the first page and the page where the latest visa in Australia is affixed.
- Three (3) identical copies of 4.5 cm x 3.5 cm high resolution photographs in photo paper (colored with plain white background) taken within the last 3 months. Photo should not show applicant in sleeveless and revealing attire; collared shirt is required. Eyeglasses should be removed. Male applicants should not be wearing earrings.
- Applicants who are permanent residents in Australia for two (2) years or more are required to submit a Certificate of Evidence of Resident Status (Form 283) issued by the Department of Immigration and Citizenship within three (3) months at the date of application. In case passport being renewed is brown or issued prior to 10 May 1995, submit any of the following Philippine documents: a) birth certificate; b) marriage certificate; c) voter’s affidavit; d) driver’s license; e) school records; f) income tax return; g) TIN/SSS/GSIS I.D. cards; h) employment IDs/records; and i) any other official IDs/records.
- If applicant prefers to receive the new passport by mail, self-addressed pre-paid envelope (preferably registered post).
- Non-refundable fee of AUD 65.00 for 32 pages and AUD 78.00 for 64 pages in cash, commercial bank cheque or postal money order payable to the Philippine Consulate General in Sydney.
REPLACEMENT OF LOST PASSPORT
1. Personal appearance of applicant for interview.
2. Duly accomplished application form.
3. Three (3) identical copies of 4.5 cm x 3.5 cm high resolution photographs in photo paper (colored with plain white background) taken within the last 3 months. Photo should not show applicant in sleeveless and revealing attire; collared shirt is required. Eyeglasses should be removed. Male applicants should not be wearing earrings.
4. If lost passport is already expired, an affidavit of loss explaining in detail the circumstances surrounding the loss of his/her Philippine passport. Notarial fee is AUD 32.50.
5. If lost passport is still valid, submit written Police Report, and affidavit of loss. There will a 15-day waiting period prior to the processing of application for a new passport. If lost passport was issued in the Philippines or by another Embassy/Consulate, applicant has to pay a verification fee of AUD 13.00. The result of verification may take longer than the 15-day waiting period.
6. If verification result is negative, the applicant has to submit an authenticated birth certificate from the Philippine National Statistics Office (NSO) and other Philippine document to establish his/her identity.
7. A non-refundable fee of AUD 117.00 for 32 pages or AUD 182.00 for 64 pages in cash or commercial bank cheque or postal money order payable to the Philippine Consulate General in Sydney.
8. Applicants who are permanent residents in Australia for two (2) years or more are required to submit a Certificate of Evidence of Resident Status (Form 283) issued by the Department of Immigration and Citizenship within three (3) months at the date of application.
9. Applicants who are not permanent residents will be subject to further requirements or interview, as may be deemed necessary by the consular officer.
AMENDMENT OF NAME IN PASSPORT (For married women who chose to adopt surname of husband)
If married in Australia
- Report of Marriage issued by the Philippine Embassy/Consulate General where marriage was reported.
- Photocopy of data page of applicant’s passport and page where the latest Australian visa is affixed.
- Non-refundable fee of AUD 26.00 in cash, commercial bank cheque or postal money order payable to the Philippine Consulate General in Sydney.
If married in the Philippines
- Certified True Copy and photocopy of Marriage contract issued by the National Statistics Office (NSO)
- Photocopy of the data page of applicant’s passport and page where the latest Australian visa is affixed.
- Non-refundable fee of AUD 26.00 in cash, commercial bank cheque or postal money order payable to the Philippine Consulate General in Sydney.
