Services to Filipinos

NBI Clearance

The Philippine Consulate is authorized by the National Bureau of Investigation (NBI) of the Philippines to facilitate the issuance of NBI clearance application forms (NBI Form No. 5), to take fingerprint impressions for the NBI clearance form, and to notarize the signature and information provided by the applicant. However, the Consulate does not transmit the NBI Clearance application to the NBI and the applicant has the responsibility to send the NBI Clearance application either directly to the NBI or a representative who can lodge the same with a receiving office of the NBI.

The requirements for an NBI clearance application are:

  1. A recent passport photograph of the applicant (white background and taken within the last three months)
  2. Photocopy of passport (the original must be sighted by the Consulate)
  3. Payment of notary fee. Refer to Schedule of Fees.


The applicant must send the accomplished NBI form to a representative in the Philippines or directly to the NBI IRD-Mailed Clearance Section. Please refer to the NBI website and to the section “Frequently Asked Questions about NBI Clearance” for the relevant details.

Shipment of human remains of a deceased Filipino to the Philippines

The remains of a deceased Filipino national who passed away in New South Wales or New Caledonia may be shipped to the Philippines only after a Consular Mortuary Certificate is secured from this Consulate. The requirements for a petition for this certificate are the following:

  1. Death certificate issued by the NSW Registry of Births Deaths and Marriages
  2. Funeral Director’s Certificate duly authenticated by the Australian Department of Foreign Affairs and Trade (DFAT), stating that the body was properly embalmed/ cremated in accordance with internationally-accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased
  3. Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease
  4. Transit Permit issued by the Local Area Health Service at the place where the transportation begins
  5. Passport of the deceased. In lieu of passport, an identification document of the deceased which indicates the citizenship of the deceased.
  6. Name, address, and contact details (telephone number) of the consignee in the Philippines receiving the remains
  7. Flight details of the shipment of the remains
  8. Report of Death to be issued by the Consulate (if deceased is a Filipino citizen).
  9. Payment of consular fee for Consular Mortuary Certificate. Refer to Schedule of Fees.

 

Shipment of cremated remains of a deceased Filipino to the Philippines

Cremated remains of a deceased person may be brought to the Philippines only after a Consular Mortuary Certificate is secured from this Consulate. The requirements for a petition for this certificate are the following:

  1. Death certificate issued by the NSW Registry of Births Deaths and Marriages
  2. Certificate of Cremation issued by the crematorium.
  3. Certificate of ash box content issued by the crematorium.
  4. Passport of the deceased. In lieu of passport, an identification document of the deceased which indicates the citizenship of the deceased.
  5. Passport of the person carrying the cremated remains.
  6. Flight details of the transport of the cremated remains.
  7. Report of Death to be issued by the Consulate (If deceased is a Filipino citizen).
  8. Payment of consular fees for Report of Death and Consular Mortuary Certificate. Refer to Schedule of Fees.
