Shipment of human remains of a deceased Filipino to the Philippines
The remains of a deceased Filipino national who passed away in New South Wales or New Caledonia may be shipped to the Philippines only after a Consular Mortuary Certificate is secured from this Consulate. The requirements for a petition for this certificate are the following:
- Death certificate issued by the NSW Registry of Births Deaths and Marriages
- Funeral Director’s Certificate duly authenticated by the Australian Department of Foreign Affairs and Trade (DFAT), stating that the body was properly embalmed/ cremated in accordance with internationally-accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased
- Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease
- Transit Permit issued by the Local Area Health Service at the place where the transportation begins
- Passport of the deceased. In lieu of passport, an identification document of the deceased which indicates the citizenship of the deceased.
- Name, address, and contact details (telephone number) of the consignee in the Philippines receiving the remains
- Flight details of the shipment of the remains
- Report of Death to be issued by the Consulate (if deceased is a Filipino citizen).
- Payment of consular fee for Consular Mortuary Certificate. Refer to Schedule of Fees.
Shipment of cremated remains of a deceased Filipino to the Philippines
Cremated remains of a deceased person may be brought to the Philippines only after a Consular Mortuary Certificate is secured from this Consulate. The requirements for a petition for this certificate are the following:
- Death certificate issued by the NSW Registry of Births Deaths and Marriages
- Certificate of Cremation issued by the crematorium.
- Certificate of ash box content issued by the crematorium.
- Passport of the deceased. In lieu of passport, an identification document of the deceased which indicates the citizenship of the deceased.
- Passport of the person carrying the cremated remains.
- Flight details of the transport of the cremated remains.
- Report of Death to be issued by the Consulate (If deceased is a Filipino citizen).
- Payment of consular fees for Report of Death and Consular Mortuary Certificate. Refer to Schedule of Fees.
